Interprofessional discharge planning and visit training with In-HospiTool (IAVI)
The development of the interprofessional training element IAVI for nurses, doctors and social workers took place in close learning location cooperation between the Higher College of Health and Social Sciences Aarau (HFGS), the Berner Bildungszentrum Pflege (BZP), the Kantonspital Aarau (KSA) and the Swiss Federal Institute of Technology Zurich (ETHZ).
